Hannah Kloch, DNP, PMHNP, FNP

Board-certified as both a Psychiatric Mental Health Nurse Practitioner and Family Nurse Practitioner, Hannah is committed to providing comprehensive, personalized care to patients. She has a strong foundation in both general medicine and psychiatry, which she incorporates into practice thanks to her Doctorate in Nursing Practice from the Medical University of South Carolina. Hannah's goal is to empower patients to take an active role in their own healthcare and to work with them to develop personalized treatment plans that meet their unique needs.

Hannah combines aspects of functional and conventional psychiatry to provide the most effective treatment for patients. She is dedicated to helping patients achieve optimal health and well-being through an integrative approach to care.

In her free time, Hannah enjoys fostering dogs for a local animal rescue and participating in outdoor activities with her own dogs. She loves experimenting in the kitchen with new recipes, traveling, and experiencing different cultures.

ACCEPTING NEW PEDIATRIC AND ADULT PATIENTS

Jaime Snyder, MSN, APRN, FNP-C, PMHNP-BC, CARN-AP

Jaime Snyder is a triple board-certified Nurse Practitioner with specialized training in psychiatry, addiction medicine, and family medicine — a rare combination that allows her to see the full picture of who you are, not just a diagnosis.

Her approach blends evidence-based psychiatry with functional medicine principles, exploring how sleep, nutrition, lifestyle, and physical health shape mental well-being. She works with individuals across the lifespan — children, adolescents, adults, and older adults — and brings particular depth to the emotional and cognitive shifts that accompany each stage of life.

Jaime's style is direct, down-to-earth, and genuinely compassionate. She creates a judgment-free space where clients feel safe being honest — because she believes that's exactly where healing begins. Whether you're feeling overwhelmed, stuck, or simply unsure where to start, she meets you there.

Her goal is never just symptom management. It's meaningful, sustainable change — built in partnership with you.

"Healing grows in honesty, openness, and the courage to speak. You are not broken — you are breaking through."

Originally from Charleston, West Virginia and now based in Charlotte, North Carolina, Jaime loves reading, hiking, traveling, biking, and live music. She's a passionate football fan and a proud Kansas City Chiefs supporter — yes, that includes being a Swiftie.
